What you send
The contact form collects your name, email address, optional phone number, chosen service, message, consent and the page used to send it. Please do not include payment details, medical records, access codes or other sensitive information. Technical request information may also be processed for security, anti-spam and service operation.
How your message is used
Your message is used to respond to the concern you submitted and support the handling of that request. Consent is permission to contact you about this message, not permission for unrelated marketing or a purchase agreement. A submission does not confirm an appointment.
Storage and owner notification
Messages are encrypted in application storage. The application retains submissions for 90 days. The website owner is notified by email through Manus notification transport; the notification can include the submitted information. Hosting, network and notification providers process information needed to operate and secure the service. Encrypted application storage does not mean that every provider copy or owner email has the same storage format or retention period.
Essential form cookie
An essential first-party session cookie supports form security, including protection against forged submissions. It is not a marketing-consent cookie. Security measures also include a hidden anti-spam field and request validation. Browser or network settings that block the essential cookie may prevent submission.
Retention, email and backups
The 90-day application retention period does not guarantee immediate erasure of all owner email, provider logs, backup or recovery copies. Those copies may persist under their applicable retention and backup processes. We cannot promise immediate deletion from every system.
Your questions and choices
Use the contact form to ask about your message, access, correction or deletion, and include your reference if available. Requests need to be reviewed and may require identity confirmation. Avoid sending additional sensitive information in a privacy request.
